Our Model
- Assessment: The Cornerstones Assessment component is a critical, ongoing process that begins upon admission. It involves a comprehensive evaluation of the individual’s mental health status, including clinical interviews, behavioral observations, and standardized assessment tools. The goal is to identify immediate needs, underlying conditions, factors contributing to the crisis, and provide a diagnostic profile. This information helps develop an individualized treatment plan, addressing safety concerns, symptom stabilization, and short-term therapeutic goals. Continuous assessment ensures treatment effectiveness and informs any necessary adjustments, ultimately facilitating a smooth transition to outpatient care or long-term support systems.
- Stabilization: The Cornerstones Stabilization component focuses on reducing acute psychiatric symptoms, ensuring safety, and restoring a sense of control and direction for our young adult clients. This process involves providing a structured, therapeutic environment where medication management, crisis intervention, and evidence-based therapies are utilized to address immediate concerns. Stabilization aims to manage distressing symptoms, improve coping mechanisms, and promote emotional regulation. By creating a supportive and secure atmosphere, the program helps individuals regain stability, preparing them for the next steps in their recovery journey, whether returning home or transitioning to longer-term care.
- Treatment: The Cornerstones treatment phase is aimed at laying the groundwork for long-term growth and change. It focuses on moving beyond stabilization, fostering self-awareness and exploring the relational patterns that contribute to an individual’s current mental health challenges. This approach emphasizes identifying core issues, exploring patterns that contribute to distress, developing effective coping strategies, and building foundational skills. Therapists collaborate with residents to set realistic goals, enhance emotional regulation, and strengthen interpersonal relationships. By addressing immediate needs and providing tools for sustained growth, the program equips individuals with the insight and resources necessary to transition to outpatient care or longer-term therapeutic support, creating a solid platform for ongoing recovery and personal development.
- Comprehensive Case Management is a crucial phase of the Cornerstones of Maine residential program, ensuring a seamless transition from intensive care to the next phase of recovery. This process involves developing a personalized plan that addresses ongoing treatment needs, safety considerations, and support systems. Effective discharge planning includes connecting residents with outpatient services, medication management, and community resources while engaging families or support networks when appropriate. By anticipating potential challenges and establishing clear follow-up steps, discharge planning helps sustain progress made during the program and reduces the risk of relapse, promoting long-term stability and well-being.
