The Final Step to Young Adult Independence
At Cornerstones of Maine, we recognize that successful independence doesn’t happen all at once; it’s built gradually, with intention and support. Our aftercare program is designed for young adults who have progressed through treatment and are ready to live on their own while continuing to receive structured support.
This program offers an affordable, lower-intensity level of care that keeps young adults connected to the coaching, clinical support, and accountability they’ve relied on—without the full structure of transitional living.

Step into Life with Freedom & Support
Aftercare is the final step in our continuum of care. It’s designed for clients who have already demonstrated significant growth and are ready for full autonomy, with just the right amount of support to help them stay on track.
With no fixed length of programming, young adults can remain in the program as long as clinically appropriate. Our aftercare program includes individual coaching, therapeutic support, and access to group community outings, all built around a real-world living environment.
Independent Living with Resources
At Cornerstones, the goal of aftercare is to provide additional scaffolding for clients as they live independently following graduation from our Transitional Living Program. Young adults are encouraged to manage their own living arrangements, medication, food, and all of the responsibilities and activities of independent living – with our continued support.
Therapy & Community Enrichment
One individual therapy session per week (virtual)
Monthly group outing with peers and staff
Life Skills Coaching
One in-person life skills session per week
Two 15-minute check-ins each week
Monthly in-home visit by a Life Skills Coach
Clients receive 1.5 hours of coaching per week, ensuring that care remains tailored, goal-oriented, and sustainable.
