Aftercare Strategies: Sustaining Mental Health Progress Post-Treatment
aftercare for young adults • Written by: Cornerstones of Maine

Starting anything is always the hardest part. And you did it. You made a commitment to yourself and to treatment. You showed up, did the work, and you’ve grown in ways that might’ve felt out of reach at one point. Now, you’re standing at the end of one chapter and the beginning of another: Life post-treatment. It can be a lot to take in.
At Cornerstones of Maine, we get it. And we believe the transition from a structured treatment environment into independent adulthood should be a process. That’s why we offer a step-down aftercare program designed to help young adults bridge the gap between intensive treatment and real-world independence.
From Structure to Freedom: Why Aftercare Matters
More freedom means routines change, increased responsibilities, and more independence; which also means more room for uncertainty. With a solid aftercare program you get a soft landing. It’s an added layer of support so your forward momentum doesn’t stall out when life inevitably gets a little complicated. Because if there's one thing we know, it's that more support means better outcomes.
At Cornerstones, that support system continues through our step-down aftercare program located in downtown Portland, Maine. It’s a program specifically designed for young adults aged 18–30 who are ready for more independence but still benefit from community, coaching, and therapeutic support.
Building on Progress: What Makes Aftercare Effective?
Personalized Plans That Fit Your Life
Aftercare at Cornerstones is always personalized. We look at where you’ve been, where you’re at, and where you’re going. That could mean school, work, creative endeavors, or just getting comfortable in your own skin. Then we help you build a support plan that fits those goals and that evolves as you do.
Continued Life Skills Coaching
Just because you’re not in primary treatment doesn’t mean we stop offering hands-on coaching in essential life skills. We’re talking about managing money, planning meals, building a routine, and more. More than just boxes to check off, these are opportunities to gain confidence in yourself, make mistakes in a safe environment, and learn to support yourself day-in and day-out.
Therapeutic Support That Meets You Where You Are
The work of recovery is a lifelong pursuit. Life is going to continue to throw you curveballs and that’s why therapy should be available to those in aftercare.
Whether you need regular sessions with a trusted therapist, check-ins when things get sketchy, or group support to stay grounded, we make sure the door to emotional and psychological support stays open.
Social & Community Integration
Recovery doesn’t happen in isolation. At Touchstone, you’ll live with peers who are also working on growth, which means built-in camaraderie and shared accountability. Beyond the house, we help you build real connections—through volunteering, community involvement, job shadowing, or simply getting out and about in the city. Life is about creating meaning, connection, and a sense of belonging. The relationships we have, with ourselves and others, is one of the strongest buffers against relapse and regression.
What Life Looks Like in Aftercare
Aftercare at Cornerstones isn’t just continued support—it’s a launchpad. It’s a space where you can start trying on the version of adulthood that feels right for you. You’ll have more freedom, but with the safety net of weekly check-ins, ongoing access to therapy, and the guidance of our dedicated team. It’s a delicate balance between independence and support, and that’s what makes it so powerful. You’re in charge of your growth, but you’re not doing it alone.
You’re Not Alone—And You Don’t Have to Figure It All Out Today
The truth is, progress outside of treatment looks like learning to live independently while continuing to grow mentally, emotionally, and spiritually. It’s about staying connected—to yourself, to your values, to a community that supports your growth. At Cornerstones of Maine, we believe in second chances, fresh starts, and slow, steady progress. If you’re stepping into life after treatment, we’re here to help you do it with confidence, clarity, and care.
